Ohioans ages 5-25, or their parents or guardians, can enter Ohio Vax-2-School once they have received at least one dose of the COVID-19 vaccine at OhioVax2School.com or by phone at 1-833-4-ASK-ODH (1-833-427-5634), ideally by the first registration deadline of Sunday, November 21.
